Office Policies
We believe that each patient is a unique individual and requires special care.
What are your appointment policies?
When we schedule an appointment for you, we will do our best to see you promptly. If an emergency occurs, you may be asked to wait beyond your appointment time. If this is inconvenient, please let us know and we will
re-schedule your appointment. If the physician’s schedules are full and you need to see us the same day you call, we will offer you an appointment with
our nurse practitioner.
Will I have to fill out forms when I schedule an appointment for the first time?
If there is enough time before your appointment we will mail patient information forms to you, to be filled out in advance. Otherwise we ask that you come to our office 15-20 minutes before your scheduled visit in order to fill out these forms. This is to ensure that we have a complete medical and family history.
What are your office hours?
Our normal business hours are from 8:00 AM - 5:00 PM Monday through Friday. After normal business hours you may call our answering service for emergencies at (517) 342-6475.
What should I bring to my office visit?
You should bring your current insurance card(s), your driver’s license or other identification you may have. To avoid confusion we request that you bring all your medication bottles with you for all the visits.
How long before my office visit should I get my lab work done if it is needed?
You should get your lab work done 1-2 weeks prior to your visit. This will ensure we have your lab report at the time of your visit.
